...now I see what you're talking about. It would appear that the Series 3 motors, used in larger trucks had this "extension" to center the fan in the radiator.
The solutions are not going to make you happy, but here goes.
It may be possible to get a 90 degree "street 90" fitting on the manifold, but it looks tight and requires the cover to be removed to install the fitting.
It may be required to install a series 1 or two timing cover to allow the by-pass to work properly. The Mopar Performance 318LA cover (chrome) is also a low profile cover and fits the Poly.
