I have a '91 Cummins W250 that I've had since new. Not too many problems to report. I've got 94,000 Km's on it now and still on the original front brake pads. The rear drums tend to stick a little, but I think that's typical of the Dodge drums. I've replaced a voltage regulator, rear brake shoes, power brake booster, and the steering column to box universal(which is a common problem). You'll never wear out shocks on these trucks, as there's no spring travel.

One thing that isn't so good is the shaking on these trucks which causes metal fatigue and bolts coming loose. The clutch slave cylinder inspection cover shook off with metal fatigue so many times I was tired of getting replacements and just left it off. Various nuts and bolts shake loose, and I had to re-weld the clutch pedal bracket to the cab as it had metal fatigue from the shaking. The body will fall off long before any major mechanical problems with the drivetrain. I agree with the other posts, the paint on these trucks is awful, and falls off when you wash the truck. Luckily, you can't go fast enough to let the wind blow it off. Maybe the paint gets metal fatigiue?

Other than that, the truck is the greatest. Mileage is wonderful, towing is wonderful, and I actually find that bench seat confortable. The running gear is great, with straight axles that are easy for front end alignments, gear driven NP-205's and it's easy to install a Braden PTO winch.
