I finally got to go through the file. This file came with the truck so I am not sure of some of
the details.
Here's what I found.

Not sure about this one: no address. phone number seems to be
636-4661 and area code may be 602 (in pencil, rest in pen)
They had pinon seal and pinion nut. Sounds like rear axle because
I know that it was worked on and the front was not.
