Even lap belts will keep the occupants from getting pitched out of the cab in an accident. Three points are better. When I get a new (old) truck, I won't let my kids ride in them at all until I install seat belts.

I use all Grade 8 bolts, nuts and washers. On the floor mounts, I drill holes in the appropriate locations, run the bolts through the seat belt mounting plate, through the floor, and then use 3"x3" minimum plate and washers as needed to keep the bolts from pulling through the sheet metal on impact. You can take a lot of clues by crawling under your modern car or truck and see how the OEM do it.

As to 3-point belts (with a shoulder harness), I've found that Saab belts are about the easiest to mount because they don't care what angle the floor anchor for the shoulder retainer is mounted, and because they bolt to the OEM points with straight-forward bolting systems.
