Yes to both answers heat and cheap brake linings will cause that to happen. Also the thinner they get the more chance they will come apart. Once brakes get hot they are not realy any good after that. Brakes are only designed for a few emergincy type stops(high heat stops) and should be replaced even if the lining is still thick for maxium safe braking power. Mark H.
