Servicing Form for Trucks

I am going to add a hobbs meter to my 48 B-1 truck since it is driven few miles, but over difficult terrain.

Just wondering if anyone has come up with a good form format for recording the servicing history based on miles and hours. And secondly, has anyone developed a servicing schedule based on hours of useage.

Any opinions on the hour meter, whether it should be a standard hobbs-type meter (ignition is on and time ticks away), or whether I should try to find a revolution meter like on a tractor (the faster the motor turns the faster the meter moves).
