Looks like NV4500. It has a granny, and a OD 5th, and mounts to a separate bell housing. Small enough to fit, nearly stock.I THINK a gm 4x2, divorced, but you'll still have to move the transfer back- somehtin you'll have to do with any OD set up...unless you use a married set up. I don't think they made one with as short of an out put shaft as the PW's, that would be ideal.
YMMV, but it's gotta be better.
