I had a '93 w-350 that had the very same thing happen to me. After a few trips to the shop, they found that the fuel shut-off valve and solenoid were very sticky. Sometimes it would stick near closed, and under the right (or wrong) situation, the truck would stumble and die, (happened a few times in PA and once on a trip in Hickory NC), at other times after this progresses, it would want to continue running even with the key shut off. They recommended a few tanks of diesel fuel cleaner and the problem went away. Not sure if this is it, but your problems and the symptoms sound very much the same as what I had.
