Along with the choke and fast idle, make sure the heat riser is working. If it's a manually-adjusted one, make sure it's in the "winter" position and the lever and flap aren't broken off from the shaft. Also, the passages in the exhaust manifold under the carb can get clogged up with soot and rust so even with the head riser in "winter" position no heat gets up there. The only way to know for sure is take the manifolds apart, but you can get a fair idea by feeling the manifold under the carb to see if it's getting warm as the exhaust manifold warms up.

If the problem is that it won't idle at all when cold, even with plenty of choke, the problem is almost certainly the fast-idle mechanism. If the problem is that it won't run without choke in cold weather, even after it should be warmed up, the problem is more likely the heat riser.
