so it wasn't the shoes after all. Still, it was good to start with the brakes properly adjusted. All that leaves is the MC, unless there's a leak somewhere. And no, the cap is not supposed to be air-tight. The level in the reservoir has to go down a little when you step on the pedal and come back up when you release it. With a sealed cap, it can't do that. Also, as the brakes wear, more fluid has to go into the wheel cylinders, which means some air has to go into the reservoir to make up the space.

Maybe it's time to put a new kit in the MC, and this time make sure it's full of fluid before moving the piston. Put some fluid on the cup and cylinder when putting the piston in too. A older guy I know had the brakes on his newish Dodge pickup done at a shop and afterwards the pedal went slowly to the floor. The shop replaced pretty much everything in the system with no luck until he asked if they bench-bled the MC before putting it in. The owner said they should have, but the kids that did the work didn't. He insisted on a new MC, bench-bled before they installed it, and it's been working fine. Also, bear in mind that it's harder to get the bubbles out of DOT 5 than DOT 3, and any bubbles will give you a soft pedal.
