I am currently bringing a Fargo from Alberta through Montana at the Sweetwater port of entry and down to San Diego, CA. The truck has not been transported yet, so this information is not proven.
1. Haul it yourself or find a company to haul it for you. Most companies will not pickup in Canada because they are not licensed in Canada.

2. Vehicles manufactured in Canada are "usually" imported duty free. Fee on vehicles made outside US or Canada is 3% cars, 3.5% motorcycles, and 25% trucks (don't know why so much for a crappy Toyota truck).

3. If you are having someone else transport the truck, give them two letters to show at the border, one stating they have permission to move your truck and another stating that you are importing the truck for your own use, not for resale.

4. A title or registration for the truck is not necessary at the border.

I used these steps: determined who will pick it up. From there determined the route and port of entry. Called 411 and then Customs at the port of entry and asked for numbers for Customs brokers. Called several customs brokers, they can fill in all details and do the paperwork for about $50.

I will stop short of recommending the transport company and customs broker I am using until my truck actually gets here (sorry for the delay Kevin), but e-mail for details.
