Somebody else can be more authoritative, but here's my take on it. There's no bolt between #4 and #5 that needs to be removed. If there is a bolt tapped into the head there, it wouldn't hurt or help if you unscrewed it (unless you break it off!), so take it out if you don't know. Different trucks have different accessories mounted atop the engine, and it's nice to be able to attach them to something other than a head bolt. Maybe your bolt is a relic of something that used to be on there. Maybe it goes into the water jacket and you need to leave it there so as to keep the water inside. You don't need to remove the radiator hose elbow. I assume you're asking because the head bolts are out and the head is not coming off easily. I've had old engines where I've had to drive screwdrivers and cold chisels in from the sides to get the head off. It can be brutal. Rust never sleeps.
