A 10" x 20" trench around the perimeter with two #4 rebar lapped 16" at splices will be adequate, 4" floor is marginal 5" would be better. Wire mesh allows a better troweled finish but doesn't do much good if it's below the concrete than fiber, either will be fine. Cut control joints to quarter the slab up. That way it will crack where you want it. If its attached to a structure that's protected from frost upheaval then footer/wall foundation is better
